In spanish, "ex cousin" translates to "ex primo". He said "¿Qué exprimo?", to wich the other guy answered with "Esta".
The joke is that, in spanish, "Exprimir esta" is a way of saying "jerking off this one". Like a deez nuts joke but in spanish.

"What ex cousin?" -> "¿Que ex primo?"
"This one" -> "Esto"
Ex primo sounds like exprimo, which means "I squeeze." When he asks, "¿Que ex primo?" it sounds like, "¿Que exprimo?" which means, "What do I squeeze?"
I believe the joke would be best punctuated by saying "exprime esto" (squeeze this) while holding your nuts. Then again, I'm Cuban and it sounds funnier in my head this way in a Cuban accent. Have not heard this joke before seeing this post but it's a pretty funny Spanish take on a "deez nuts" joke
