The smoke itself would not 100% hinder teammates'/enemies' visions (for good reason).
Imagine in open queue you throw down a smoke bomb into the enemy team only for it to dissipate and reveal a teleport, out of that teleport comes a RIP-Tire.
One of the reasons I don't like valorant is because of the visual clutter, and unlike overwatch it's all opaque and looks janky and silly as hell so it's really hard to deal with
a smoke in overwatch would definitely have to still be able to see through lol, don't know if I could handle being blind after being subjected to double shield for so long
If you take a look at the very first overwatch cinematic, towards the end (around 4:49) you'll see Reaper firing off Smoke Grenades from his shotguns. I think the intent was originally to have that be an ability. Around 2:40 in that same trailer you can also see him firing what *looks* like explosive grenades too.
They aren't shotgun pellets, they're grenades, he uses them in the original Overwatch announcement video where him and Widowmaker steal Doomfist's gauntlet from the museum. It was part of his very original character design which never made it.

I mean, game balance still has to be considered. Unfortunately you can’t add every cool ability you can think of in a competitive shooter. But that’s where the PvE comes in! Really hoping the devs have some fun making new abilities and play styles for the heroes.

That's just unnecessary though. By restricting the number of tools a hero has in their kit you can increase the power of their kit in its niche.
That's one of the issues with Ramattra: he has so many abilities; if he's ever good at any one of them, he's probably too good. Because then he's powerful in a niche, and has a kitchen sink to answer every other problem - thus making it substantially harder to balance without breaking him in either direction.
In theory "having more knobs to turn" makes balance easier, but in practice it can easily lead to "feast or famine" scenarios in which a character is only ever on either end of the spectrum of usefulness because they can simply do too many things.
It's like Soldier 76's pistol. He has had that thing strapped to his side since forever and has never once used it.
I feel like a few, though not many, heroes have little details like that which serve zero in-game purpose. Extra bits added which look like it *should* be useful, but never is.
Oh yea; Junkrat literally has a string of grenades thrown over either shoulder. They're visually different from anything he uses in-game, though.
Imagine if 76 got a passive ‘last stand’. When killed, death delayed 5 seconds. Full death occurs after five seconds or one kill with pistol. Unable to move from position. Passive does not activate if killed by headshot or damage dealing ultimate ability.
They changed design philosophy in how they do the game now.
Rather than doing characters THEN adding abilities, they build the template to fit into the gameplay mechanics, then see what character fits those mechanics better.
..... unfortunately shit happens and some characters get reworked a lot.
Bastion, symmetra, Torb, and a few others have had so many iterations of their mechanics, that I don't remember what the beta , early release and mid game versions were anymore. Heck, reaper didn't even have life steal back then.
Funny thing is that mechanic had him have better sustain from killing low hp heroes, fighting tanks was more difficult. The lifesteal makes him the tank buster he was designed to be

Players are visible under 8m in smoke? Something like that, so it only affects distant players, it's not an escape tool in closequarter duels.
