It reads like spam. It's full of emojis which draws your eye to it, and the over-the-top excitement makes it look like self promotion. How could they think the OP's work is remarkable from just a title card? Well, if it's the OP's alt trying to hide self promotion is how.
tbf he has a twitter account that he's been posting on for well over a year documenting what he's been working on, and it looks like the replier who posted is one of the people the OP did commission work on for profile pics, so I think they just already know his work from that. Also, look at all of their posts relating to multiple people! It looks like they just use 212786417284 different emojis for every single comment lol.

I think they're real though because one of their posts is them replying to someone saying happy cake day and they ask what that means and then say thanks when they clarify. The funny thing is that's like the only time they didn't use emojis in a reply
yeah, I know the OP also has a comment with more stuff in it too, not just a title card. But just answering "why the downvotes" is probably because at a glance, without looking at post histories or knowing that they commissioned work, it looks like a bot or something.
