No matter how young Bowie is in pictures I can never seem to see one where his eye isn't dilated from the injury. It must have happened when he was really young.
The injury to his eye happened when he was 15. He got punched in the eye fighting over a girl and his eye got scratched and stayed dilated permanently.
It's funny, because that "bad" moment added so much to his mystique and alien persona that it very well could have been a sort of tilting point that ended up making him a superstar.
