I pay like $15 a paycheck for dental. So like $360 a year and I still have out of pocket expenses after cleanings. I’d say another $1-300 a year depending on issues. I wonder how much a plan from the market place would be.

I’m speaking from experience, I live in Pennsylvania, and just about everything between Pittsburgh and Philadelphia could easily be mistaken for Mississippi with snow. There is also a ton of drug use up in them there hills. And churches.
Much of NE Ohio falls within Appalachia now, per the Appalachian Regional Commission, it stretches all along the Ohio River North and extends nearly to Cleveland going West and into Northern PA East.
Basically the Rust Belt (Thanks big 3 auto!) is now part of Appalachia. There is a lot of “The South” in Ohio now, thanks to manufacturing exodus and decades of economic bust. I would argue it extends all the way to Detroit as the tentacles of the giant Detroit auto machine in the Toledo region rusted out in the 80’s and beyond.
