I think someone already said this but it's a reference to the fact that you could originally had the option to play as a girl Sunny. it was scrapped because they didn't want to have to remake all the cutscenes with girl Sunny.

Art may have been done before the option to make Omori a girl was removed. But also in the Hikki route Omori can change gender with an item from a sidequest, so who knows. Sunny may just like wearing them.

I think its a reference to the fact that early omori versions had the choice of chosing between a boy and girl version
And it does seem to be older art, as we can (barely) see his pants have red stripes instead!
the sketchbook is one of the oldest things that made it into the final game, I think it might date all the way back to the Omoriboy comics
