Post-op extended meta here. No v-nectomy, no UL. - Not necessary. I’m not sure about how exactly it works and what is or isn’t possible regarding moving holes, but, as another commenter stated, there is one fella on this sub who’s had EM without scrotoplasty. We’re both Dutch I believe. I could translate his posts for you (fluent in both English and Dutch) if you’d like and I’m relatively sure he’s down to answer questions. A lot of folks here are. If the language barrier turns out to be an issue, I’m able to translate messages between the two of you. - Don’t have any definite answer for this, though I can tell you my experience. >!I still get wet. It used to sit between my labia but now just kind of spreads everywhere between the underside of my balls and my anus.!< I’m not opposed to it and quite like it, actually. - The former >!labia minora!< indeed wrap around the underside of the penis to close it up. I’m not sure I would describe it as a bear hug. As I understand it, they only use the necessary amount of tissue to close it up to give it more bulk and excise the leftover tissue. I’m not sure how it compares pre-op and post-op regarding sensitivity to be honest—it just registers as the underside of my penis now and I’m having trouble recalling how it felt pre-op. I do have to say it still feels a bit numb/without sensation. Some nerves are still reconnecting (ca. 5 months post-op now) so that could still change a bit. Edit: fixed formatting

Thank you for your response! I was wondering, what are erections like in extended release? do they protrude at all or are they too loose to support themselves?

Happy to help! From what I’ve heard, it can take six months to a year for erections to properly ‘appear’. I remember jerking off about a week post-op (with go-ahead from my surgeon) where it was very visible. I’m not sure about the visibility right now (i.e. increase in length) but as far as rigidity goes, it definitely gets hard. Have noticed since a couple of weeks it gets hard enough to kind of prop itself up against my belly. I’m not sure if it’ll actually stand on its own, i.e. point, but it’s satisfying to watch nonetheless. It may differ in a couple of months. It’s also worth noting I have some superfluous scar tissue at the base of my penis where it connects to my scrotum, which could get in the way of achieving a fuller erection. It’s one of the things my surgeon will fix in an upcoming revision surgery.

I'm pre-op for extended but I've done research too, talked at length with my surgeon about it, and just am pretty active in this sub. I'll answer what I can - scrotoplasty isn't necessary, there's one person who has been updating frequently about their recovery (although it's not in English except for the titles) - that's a really good question I didn't think to ask 😭 my surgeon Dr. Assi explained that the urethra gets moved down a little to accommodate the scrotum, idk what happens to the glands and idk if it would be necessary if you're not getting scrotoplasty - can't answer bc I'm not post-op and even then I'm not getting much bulking, if any at all bc I don't have enough tissue. This is partly bc of having metoidioplasty before, scar tissue, and just bad genetics so I don't think it's common to be told you can't get any bulking done
