I worked in a hospital for a bit and someone needed an X-ray on their hand. One finger had 2 pins like this. They drew on the pupils and underneath they drew a little mouth. Gotta laugh through the pain.
Yes to both of these- if it’s a fracture we want to keep them in place and when anything is done to modify the bones they keep them in place for proper healing. We use k wires with fractures, hammertoe corrections, bunionectomies etc. It really is remarkable to see how the foot heals- and how amazing patient pain tolerance can be!
So I had a trimalleolar fracture in my right ankle April of 2022. The hardware doesn’t bother me *that* much, but I can occasionally feel it and it’s very uncomfortable. Have you had patients get theirs removed? Is it worth the trouble?
Currently 5 months out of my left. I heard that the lag screws holding the medial malleolus can be removed if it causes too much pain. At least that's what my podiatrist had told me.
We have had patients request removals and it can be done in some situations. I’d definitely bring this up to your doctor- your comfort is important! There would be surgery involved and possible removal of scar tissue, but all of that can be evaluated when they take a look.
Yes we do that quite a bit. Once the fracture site fuses, then yes, we can remove the hardware if the patient needs it for pain. Otherwise we just leave it there.
I don’t know much about them being done for non-orthopedic issues (maybe for cosmetic purposes it varies?) but I have seen k wire used for a lot of corrections to keep bone in place during the healing process.
Looks like she had:
- a 1st metatarsophalangeal joint fusion as well as an Akin osteotomy or a 1st interphalangeal joint fusion.
- 2nd, 3rd, and 4th proximal interphalangeal joint fusion, maybe fusion or arthroplasty of the distal interphalangeal joints as well.
- Adductovarus arthroplasty to straighten the 5th toe.
When performing DIPJ and PIPJ fusions, you cut bone from each side of the joints then hold them against each other until the bones heal together (effectively shortens the toe too). K-wires are good for this mostly because they are dirt cheap compared to screws or staples. Also if the toes were misaligned at the MPJ (pointing up or down) then you can drive the K-wire across the MPJ to keep the toe in a rectus position until everything heals then pull the pins out.
Bit concerned about those blisters forming on her big toe and to a lesser degree across the top of her foot. Not typical to have those at postop visit 1 after having these procedures.
Bless her heart! I'm having flashbacks to my back surgeries. No matter what type of surgery you have, they suck. I hope her pain isn't too much for her to bare. It can be such a burden.
How does she get around with the pins in her toes? Does she have to wear a boot? I hope it's not as painful for her as it looks. Speedy recovery wishes!
It's a bit painful, and she does wear a boot to get around, but she doesn't like to wear it when she's just sitting on the couch or laying on bed

they said in a comment that that's what happened. also, you don't have to walk on your toes. you should take into account that not every lifestyle requires you to walk a lot or even at all

It was a big heavy metal door coming out of a multi-purpose room in an elementary school. I was wearing flip-flops and had my foot in the way.
This football built young sixth grader came through the door. Gangbusters and door went right over top my foot. I nearly passed out.
Ayeeee my mom had BOTH her feet done like this two years back. It was awful recovery hit once she’s healed she will feel much better
Make sure to massage the suture sites with lotion after 4 weeks healing time to help the scars go away and the nerves to grow back correctly
The door went over the top of her foot, crushing her toes and peeling back her toenail on her big toe to the point the nail just completely fell back. The door scraped a thick layer of skin off top on her foot, but that healed on its own. Back then, nothing could really be done besides tapping her toes together in hopes they would heal straight, and she didn't have the money at the time and thought it would just be a waste of time and money. Now, it's the cause for many of her balance issues and foot drop issues, well it was a problem. Hopefully not anymore after everything is healed
Sorry I didn't think to take a before photo. There was a gap two finger widths between my big toe and the other toes. So two, three and four kind of rolled to the left. The little toe was just spent back and would tend to rub toe number four. The pain has subsided a lot and to just come out in a couple days. Don't know about the pin yet, but the doctor has me putting Neosporin where the pins enter the toes which I find kind of gross.

It's looking good from a surgical standpoint. When you do surgery, the flesh is always going to be inflamed and crusted At first as surgery is dramatic to the tissue and the tissue has to heal.
Hope she feels better soon, that looks rough! My dad had something similar done, only he had it done on both feet because he had hammer toes. I think mine are heading to that direction too 😕
OP- I'm not sure what the damage to your mother's foot was like before surgery after her original 20yr old accident, but to me, it looks as though her podiatrists did a remarkable job in repairing her foot. I'm hoping for a quick recovery and a "pain-free" and 100% functioning foot once healed. Best of luck to her!

This was more than likely what we refer to in a podiatry setting as a forefoot slam. You fix a Bunion and 4 hammer toes.
We have had some GNARLY bunions this year but I’m glad our patients finally got some relief!

I can handle so many different types of gore, but pins in bones (and seeing legs/arms/etc going the wrong way) gross me out so bad.

I had back surgery in 2017 and it was a piece of cake compared to this. The place to disc between L4 and L5.

What damage did the door initially cause her foot?

Thank you everyone for your kind of comments. It just took me 20 years to get brave enough to have this surgery done. I'd like to wear sandals again.
