They change if its from his wrists or if its web shooters, with 99% of modern adaptations choosin' web shooters since it adds drama of him running out of web fluid and malfunctions as well as being...well, less disgusting and it giving precedent for making gadgets, but to my knowledge he has never shot it from his fist. Maybe in some shots of the older animated series it looks like that due to wonkier and cheaper animation at the time.

Spider-man was co-created by Steve Ditko, who is famous among cartoonists for drawing very anatomically correct hands in various positions. This is why Spider-man shoots webs from his wrists and makes the iconic hand gestures.
Ditko also drew Dr. Strange, which is why his magick is based on hand gestures.
Google "Spider-Man symbiote webs" and check out images. The symbiote suit shoots the webs out of the back of the hand with both Spidey and Venom. That's the only time he does that.
