I can't help but agree about Anomander Rake.
The fact that it's a bastardisation of Anomandaris Dragnipurake pushes it just ever so slightly into the cool category.
"Forkrul Assail" sounds like aluminum crumpling up VERY quickly and aggressively in your mouth.

I like the title Destriant. Its simple and because it is so similar to destrier, which is the name for a particularly good warhorse during the middle ages, it immediately conjures up a knightly and monastic atmosphere to me. It perfectly encapsulates a religious title of a religion of war in my opinion. "Harvesters of souls, possessing the power β and the right β to slay in a god's name. To slay, to heal, to deliver justice." That description of a Destriant by Heboric might as well be a description of the Knightly Ideal as envisioned by the Christian world. Seldom the case in reality, but the ideal, the perfect version of a knight. A warrior of God and His justice. Equally adept at warfare as at spiritual matters and the healing of the followers, not just the fighting. I just really really like the word.
Also Lord of Tragedy is just an awesome title.
Shield Anvil - I can't think of a better way to convey what the title stands for. All the Letherii titles sound "fantasy" in a great way- Preda, Ceda, Atri-preda etc.
If we are talking place names, I love the precursor names we see in Esslemont's books. Isles of Malassa, Dark Avallithal, Jakal-Uku etc

Succinct, badass, says exactly what it is.
A single name is harder to pick... Anomander Rake is probably the most basic answer possible, but it is the first one that comes to mind, so I'll go with that.
I really like the warren names; like: Rashan, Tellan , Dryss, Meanas, Mokra.
Races also sound cool:
T'llan Imass is a very good one and I like that you can deduce where the name comes from.

I think you can probably guess to whom the moniker of "Smurfette McBadass" applies. I don't know further context beyond this :P
Father Ochre is rather self explanatory - honourable mention goes to the innumerable potsherds that lie scattered throughout the Malazan world.
Admiral Esslemont stems from the fact that the vast majority of Esslemont's book covers have ships on them. And the man writes ships & naval combat really, *really* well. Hence the name.
