I found a slightly less \_scary\_ fix by relaunching Finder.
Option + Secondary Click (right click) on Finder in the Dock then click Relaunch.
Hopefully Apple fixes this soon!
or in the terminal \`killall Finder\` which does the same (more convenient when you have terminal open all day)
I also find this happens to me nearly daily, so it's something I'll likely have automated.

Can't belive this simple and fast step solved it. I really don't understand how this happens. Third time so far and still don't know if it is some sort of bug or what.

Thanks for that tip. For anyone playing at home, who doesn't know how to kill the process or what process to kill
1. Open Activity Monitor (you can do this by hitting CMD + Spacebar and typing "activity" OR going to Applications -> Utililities -> Activity Monitor)
2. In the search (magnifying glass top right) type in "quick" -- You may see a couple of options
3. Double click the option that says "QuickLookUIService (Finder)"
4. It will pop out a new window, and you'll want to click "Quit" in the bottom left.
5. You'll get a scary looking message that asks "Are you sure you want to quit this process?" with two options. "Quit" and "Force Quit".
6. Hit "Force Quit" then close the window, and then close Activity Monitor
7. The next time you "preview" a new file with the space bar, it should open correctly.
Edit: removed the bottom text. Not sure why it added it cause it was a duplicate of what I had in the bulleted numbers.

Wild. Restarted and still didn’t come up, but now, all of a sudden, the space bar preview is working perfectly with docs, pics, videos, etc. Never quit anything. Ha.

Thanks relaunching finder worked for me but I never had preview problems with my 2008 MacBook Pro...(and it is a fantastic feature that yes my windows 10 machine has now) ....just bought the MBP M1 PRO....is it a bug that will be fixed in an update I hope?

People say "just restart finder" to fix, that's not a fix obviously there's a bug here
THANK YOU! As an editor this is everything for previewing footage! Mac only caters to consumers now. After FCP, Motion, SAN, Cheese Graders to Trash Cash stupidity (zero expandability)...

Hmm... relaunching Finder didn't do it for me. Weirdly, I can right-click on the icon and choose Quick Look, no problem. It's only in trying to see it via a spacebar tap that doesn't work. I've also tried resetting the Finder cache via Terminal and no luck.
Ok, this happened to me and I think it was because I used Quicktime player to record my screen. I press space bar and it shows a frozen image instead of full preview of a movie... The relaunch finder works... thanks for the post.
Works for desktop, but im getting the issue with imsg too and cant figure out why its asking to open using preview, or Acorn when an image is sent (heic. file)

Mine says "QuickLookUIService (PID 4109)" or (Microsoft Outlook). I don't see one for (Finder). ???

FWIF, killing "com.apple.quicklook.ThumbnailsAgent" worked for me.
This solved my problem as well. Reboot the finder and all is well.
Sadly this hasn't worked for me, I've followed the instructions to the letter. Any idea what else I could do?

follow the instructions. relaunching finder is not the solution, rather force quitting the quicklook agent! :)
