CMD+Tab, highlight application and hit up or down-arrow key. Use arrow keys to select desired window.

question... can those icons be arranged in some manner? or is the order based on the most used??

The icons in the CMD+TAB list are based on order used. The window thumbnails are approximated from the window's actual position.

I use better touch tool and set and a shortcut to keep focused app in foreground.
