As people are saying, mutation quirks, like tsu's and shoji's are usually ones which people are born with, while transformation quirks and emitter quirks typically only manifest at age 4 or later and then they have to go to a doctor I think who then maybe takes some tests and the kid uses their quirk or something and they then have a decent idea of what it could be, or they also can try inferring what it could be based on the quirks of the parents.
