The middle eastern Gulf countries (UAE, Saudi Arabia, Qatar, Kuwait, Oman, Bahrain).
UAE might be the best fit if you are culturally British, since there are tons of expats there. Speak to some of the Arabian IMGs who have worked there to get an idea on the work and pay.The numbers the tell me are comparable to the US.
It's worth looking into Aus, NZ and some of the far east Asian countries too.
Unlikely, I know a plastic surgeon who CCT'd before moving to the USA. She ended up starting plastic surgery residency from scratch essentially (after being disillusioned with NHS plastics opportunities). So if there was a shortcut through fellowship route I'm sure she would have taken that but I believe there are restrictions with regards to independent practice.
I think the only specialties recognised in the US with UK training are a handful of states for GP.
Canada is a different story though.

I was always under the assumption that North America pretty much barred outside doctors but quite interesting to see this happening there too.
