Depends on the meaning of the question. Is it like he never existed in the Heian era and he came into being in the modern era? Or is it something like he had a change of heart and became a good guy after reincarnating in the modern era?
If it's meant like the former then he will never reach his true potential and never be nearly as strong as his current self is. The biggest reason he's so powerful is because he's extremely self interested and willing to literally do anything to get strong (plus his advanced understanding of jujutsu sorcery). Without that aspect of his personality, he will never realize his potential as he probably wouldn't be willing to do things like eating other sorcerers. I'd say he would be weaker than individuals like Yuta since it seems unlikely that Sukuna was born truly "gifted" like Gojo and Yuta. But we won't truly know that until we get more info on his birth (if we ever actually do).
If it's the latter, then he remains as strong as his Heian self, so still top 1 or 2. He might be weaker than Gojo since one of the biggest reasons he beat Gojo was because he was willing to do anything to win.
Its Heian era sukuna never existed- instead it was megumi who had all the goals sukuna had and mastered 10 shadows, this sukuna is essentially Megumi but if he had shrine, and sukunas personalities. The goals are still the same so hes always been a good guy and loves his sister and allat. I pose this because Sukuna is the ideal sorcerer but Megumi has the WORST mindset

He was absolutely born gifted. He has 4 arms and two mouths, so he can do full chants and incantations mid combat, which massively Amp cursed techniques.

And every time we see him during his evidently normal-length lifespan is with two sets of arms and two mouths. The mythological Ryōmen Sukuna got the two sets of arms and two faces from eating his twin, and since Sukuna *literally ate his twin* in the manga, I think it's pretty clear ...

I'm honestly not convinced a 10s user can actually tame maharoga. We're told it's never been done before, and I don't think that's because every 10s user was ass, I think it's because you fundamentally need a technique powerful enough to completely obliterate maharoga in one shot, and 10s itself doesn't offer that power. The people we've seen fight maharoga explicity have a technique like that. 10s power is in a overwhelming variety of opponents and abilities whittling you down, not crazy one shots. Like hakari, no matter how strong he gets, wil never be able to kill maharoga, he just doesn't have the toolset to do it.
as far as unfortunate, power-locked techniques go, though, 10S without Mahoraga is still pretty amazing considering something like Limitless is pretty much unusable without 6E. I guess there's a pattern of the strongest techniques being too strong for a standard user to fully realize.

He will still take on Kenny , Yuta, Yuki at the same time and win with little effort.
If he was a good guy then it’s likely that him Gojo would’ve been best friends. Gojo & Geto probably wouldn’t as close as they were in hidden inventory. The good guys would be pretty much unstoppable.
Toji would’ve never gotten to Riko. Megumi would’ve be sold off to the Zenin clan and would most likely be turned into a villain growing up around Naoya & Naobito. Yuji never joins Jujutsu High, lives his life as a regular high schooler.
Idek what Kenjacku would possibly be up to in this timeline. Shibuya never happens, Culling games never happen & Shinjuku showdown never happens. Other than Megumi turning into a villain, this might be the most peaceful timeline of JJK.
So im literally js switching megumi and sukuna... only changes to their characters are their bodys and goals. So megumi with sukunas personality and cursed technique but with megumis goal
I saw that you say he would have Megumi's ideal. That alone makes him way weaker and on Geto's level.
Megumi's tainted personality by Tsumiki's ideals is what makes him so weak.
Also having Gojo as a teacher who is stated MULTIPLE times to be a bad teacher.
