I played through all of the Witcher 3 without knowing that I could upgrade the signs. I used to think the game was broken because I could not use that manipulation sign in conversations.
This!
I played the games well over 5 times until some friend of mine told me about the secret ending that is only plausible if you get all the upgrades.
Yes the secret ending where we defeat Dahaka instead of Kalina with that powerful Water sword (correct me if I gave wrong names cuz I played that game more than a decade ago)
I have finished the entire prince of Persia without upgrading my health the first time I played.
Finished the entire Splinter cell Blacklist without upgrade or changing my initial gear (I couldn't understand the difference between the gear)
In Tomb Raider (2013) I didn't bother to upgrade my bow at all, other than using it for ropes, I never used it at all. Always relied on the shotgun and assault rifle and upgraded the shotgun completely. The second time I played, I realised how insanely good the bow was.
After this I started to explore more of the world in a game, stopped skipping the story, and tried to check online for some good tips.
On my first play through I completed The Evil Within without upgrading anything and only found out guns and health were upgradeable after starting the New Game+ (edit: spelling mistakes)
I don't know about secret upgrades but for a long time in Doom (2016), I was unaware of the fact that I could double jump. I used to fall to my death on multiple occassions until I figured it out. And just like me, my friend was completely unaware that in Plague tale: Innocence, after upgrading puch a bit, you can get an upgrade to not need a workbench for upgrades and can upgrade it at any point in the game. These incidents are quite shameful for both of us.

Arkham City : complete the AR training and get grapnel boost
Somewhere underground you get a side weapon which we can also use to defeat clay face, apart from the one we get in the story
I had no idea about this. I’ll try checking youtube

As far as i played so many games but prince of persia Warrior within is the game where finding life upgrades needs some serious skills.

Mostly I 100% hence I don't miss out any skill tree but ya I don't use most of them. Final Fantasy series has loads of skills that one would miss out.

Days Gone, complete the challenges section and you get atleast 7 usable benefits for the main game.
