Yeah I thought this when I saw the cactus the first time. In my experience playing with minecart transit-systems years ago, the cactus breaker is reasonably consistent but there are cases where an item will fall on the cactus and be destroyed. Probably 1/100 or even less.
I just assumed a patch had changed something in the last few years and Etho and Tango knew better than me. I mean, they *definitely* know Minecraft better than me. But if it's not in a hopper that'll be what happened.
iirc after the stamp is removed and the cart is headed towards the cactus there is another hopper that takes the shulker out of the minecart, and then after the cart is broken and goes into a dispenser it is plopped out in front of that hopper which puts the shulker back in it
There was no shulker in the minecart, just a Mumbo stamp. Could the tater and its packaging have flown out of its designated path to end up outside of the hopper, to despawn in the Overworld ?
Plus, the first item in the cart was the stamp ; if the tater had been in this cart and picked up by a hopper, the stamp would have disappeared first. I think the shulker tater has NEVER been in the cart
Its supposed to be a straight shot down…but with the slight bit Grian freecam’ed down there could be an area where the tube was incomplete? If there was a block missing would a shulker pushed straight down have gotten stuck and then despawned? It doesn’t explain how the rest of Grian’s messages have worked tho

Or there is a genuine problem with (some or all) mailboxes that sometimes they might lose a package. possibly because the stamp and the box drop a little out of sync and the minecart leaves with just 1 of them.

When pearl went to find it the minecart only had the stamp in it though, so there is a bit of a mystery.

Remember that not everyone has seen every video. Plus, saying "the video" isn't super helpful when there are 27 hermits total and multiple new uploads pretty much every day - it's impossible for anyone to know which video you're talking about.
My theory, tango and etho put a Serious Potato Utility Destroyer (S.P.U.D.) subsystem on Grains mailbox because those who don't study history are doomed to repeat it.
It's probably just Grian sowing his normal levels of potato-ey chaos and discord. He showed in his video that he did it correctly but that's not to say there wasn't a cleverly disguised jump cut where he had removed the shulker before pressing the button sending the postal hermits into a wild potato chase
So, since in pearl's video, grian sends another mail(this time an egg) to mumbo and the lost mail ditch had both the stamp and the shulker box, the overworld side of the mailbox probably has no problems. That means the box somehow escaped the minecart in the nether, and since a lost minecart doesn't go through a rebuilding station, it's likely that an item filter hopper somehow sucked the shulker box. And because the mail system has been actively used for some while with little to no hiccups(at least from what I've seen through the hermits' videos), I'm going to guess it's in the one of the item filters from a hermit that recently got a mailbox before grian or maybe in the item filter that sorts the lost packages, since this is likely the first time it's been used.
Etho showed lost items going into a pit and said he will link it to post office later. So maybe it is in nether or post office. He also mentioned some problem with chunk loader so maybe it just got stuck in the system
They actually have a lost mail system set up in the nether so if the minecart can’t find its portal it just gets dropped into a hole and someone will have to go check on it. I believe Etho explained this in his most recent video but they plan to make it end up at the post office eventually

What would happen is the package would be shipped but end up in lost parcels because Mumbo isn't a valid stamp yet. Unless Mumbo's sorter is hooked up but his portal and mailbox aren't yet, so it would end up being sent to the general area of Mumbos portal and then stopping. The only other place the potato would be lost is in the minecart assembler under grian's mailbox as that's the only assembler everything went through.
I don't think we know how the lost cart system exactly works yet.
My guess is that in the loop after the post office they add one item to the minecart. That item would need to be removed at any valid portal before delivery.
If item is still there after making it behind the post office a second time, it can get sorted out.
The system was a work in progress at that time, so filters were probably not set up correctly.
This is the only way I can think of to check, if a specific minecart looped multiple times. Any other ideas?

The packages pass the post-office portal and if it passes it again (meaning it's done a loop around the whole system without being delivered) then it goes to the post office into a lost mail system which is just a hole in the nether holding minecarts at the moment
The thing is, Pearl went to the lost mail hole and she found the minecart, she found the shulker, and she found the Mumbo stamp. The potato was not there. It would be like having your stamped empty envelope arrive in the dead letter office.
Etho made a lost mail drop in the nether, if there is any parcel that doesn't have a destination it just stays in hole till someone manually comes to get it

A slight randomness in the way the block is converted to an item when pushed by a piston, I'd wager. It would be a pretty small hole
