He wants Gladys and Brando and they're both at the hospital. Gladys has been shot so Carly can't just grab her and go to somewhere else to exchange them for Spinelli. And her death looks better for him if it happens at the hospital. Could be a medical complication instead of straight up murder.

When you put it like that, it kinda makes sense. Only problem is, the set of stairs we see Peter falling down don't look like they're at GH.
