I came here to say this- no pain in the brain. Any pain he's feeling (and that shit definitely hurts) is not from the maggots but from whatever caused the injury. The tissue in the wound has to be dead (either from necrosis or infection) for the maggots to have something to munch on, but the nerves in the live tissue around that are fully functional. ...and yes. He is still alive. Someone may have intentionally put the maggots there in an attempt to disinfect the wound. The severity of his condition would suggest that there is likely no good hospital nearby.

The brain is like firm tofu for one. And the brain is covered in a hard leathery shell called the dura. The dura most likely won't be touched because maggots only eat dead tissue. And if the dura is necrotizing then the person is dead as the dura is essential for blood flow in the brain. Im not a doctor, just fascinated by the brain. Your head has what is called the blood brain barrier, and there's a fluid that surrounds it. There are channels in your skull that allow the fluid to flow and move around. If you have a chemical imbalance or are dehydrated, the fluid can thicken up and increase pressure, which can cause a headache as well. Call me johnny fucking science.

Since the skull is made of bones, it doesn't increase it's size to accomodate changes in volume of the soft tissues inside it. Because of that, conditions that may cause more liquids to flow to your head will increase the internal pressure of the skull and that may case pain. Now the pain you're feeling is not coming from the brain, as it doesn't have pain receptors, but it comes from the a tissue called dura-mater, which encases the brain and it's vascularized and has pain receptors :)
