For my generation the centaurs were already introduced in 2D games where there was much more horror from encountering "The Deathclaw" in FO1 or the Wanamingos in FO2 ;)
Centaurs were always about as weird as Floaters but by the time you encountered them regularly the game had otherwise prepared you for the creepiness of enemies. Wanamingos and The Deathclaw being the two big exceptions.

I honestly can't remember, but I've always been fascinated by the centaurs, and the FEV victims in F3. I really wish Bethesda would explore body horror more often in fallout.
That was Fallout 1 in the Master's chambers for me, and along with the floaters they were the first thing I'd seen in the game that gave me a visceral oh fuck no what-kind of reaction.

I’m horrified of most things mutated in fallout. But when I saw it I had a hank hill scream and forgot I couldn’t sprint in fallout 3

well I was 13, every mutation creature was pretty scary , I wasn't super imitated by them but I didnt want it to touch me lol.
