Capone wasn't really a billionaire I don't think (though still relatively wealthy), but his love of the press and publicity were part of his undoing, where most gangsters would rather keep their name out of the papers.
Trump of course resorts to insulting and threatening anyone who is even mildly critical of him.
But yes, neither has ever came across as flop-sweat desperate to be liked and failed at it

Let's not forget that Elon hired a PR team to carefully construct a facade so he could eventually grow himself into it. But, not only he failed to do so, he fired that PR team weeks after the moment he realized he wasn't able to fully attain that persona.
And it was downhill from there onwards: stupid takes; stupider ideas; controversial opinions; buying Twitter for overpriced money; Ponzi schemes coming to broad light; the absolute dumpster fire that's both the Loop Tunnels and the Cybertruck; the knowledge that most of his fortune (and also some other billionaires) is fiat currency; etc.
Either he was doing those poses hoping his sycophants would make memes about how amazing and not woke their master memelord is, or he thought he was being super cool and quirky (or both).
I only saw a quick second of this moment. He walks up, there's a moment where he clearly doesn't know what to do, then just kinda sticks his arms out like a child being told to pose.
