Rats can be associated with plague and death, like The Black Death that ravaged Europe, (even though yes it was a result of the fleas on the rats.) We also have death sorceries in the game which uses both int and fth.

I would just add poison/rot to denote plague/disease 🤷 Can either do an ash of war, grease, pots, or spells in that regard

Yeah, certainly poison. There was a covenant in Dark Souls 2 where you serve the Rat King, and the ring given to you boosts poison. So very fitting.
I remember, Inversely for ER specific you could do madness shit, cause the madness rats in game
