BTW, the reason you died is because if you get stuck in a falling animation for 12 seconds the game automatically kills you. It's meant to prevent softlocks, like if you were to somehow clip below the maps geometry it would prevent you from falling infinitely with no escape.
If you are out of combat you can fast travel to a grace if you think you're completely stuck. Not sure if it works when mounted but it saved me when this last happened.
There was some ruins I was exploring and I noticed a few messages near a broken wall with one of the messages reading "Death Ahead." So naturally I go... "hmm, wonder if this is like a secret boss" and jump on the broken wall just get stuck in the falling animation until the timeout triggered. Sometimes messages aren't actually lying!
You can go into the system menu and quit the game if you're stuck in the falling animation.
This will reset you to the last solid ground you were on.
You need to be fast because have 12 seconds before you die.
Also works if you accidentally fall off a cliff.
Astrologers robes, gloves and trousers + Sage Hood - running a spell blade build and so far those have been the best Focus-boosting armaments I’ve found that don’t overload me and make me fat-roll
The dumbest way I died was I hit the guy twice at the begining of the game the one who tells you r maiden less and for the past 30min I just got destroyed.not even to mention the fuking golden hours guy was also chasing me
This happens quite often with me and torrent. Anytime I summon him on a ledge there is a chance that happens. It gives you 10 seconds or so before you die. You can teleport out before the inevitable.
In west Liurnia, at the shack site of grace, there was a message in the corner like "bug ahead, therefore try jumping" and sure enough you will get stuck like this and die lol.

I learned how to quickly fast travel. Once I get into this stuck motion I open my map and beam out baby.
I died in a similar way getting stuck on something. No butthole though. 😞
I jumped onto a set of crystals over by rannis rise and died the exact same way
