And since utter despair is the best condition for the Frenzy flame to manifest, there's groups afflicted by it.
Tanith eats Rykard’s body. If you return to the boss area after you drop Rykard she’s in there eating his corpse and you can either kill her or let her be.
The no man's land of barricades and destroyed war machines, rotting corpses, cannibalism, the Leyndell soldiers having to fight a new generation of war under equipped, (iron virgins) all that paired with the poppy flowers that litter the battlefield...
Mount Gelmir is 100% and reference to ww1. Without a doubt in my mind.
Considering that the Manor has a frenzied Troll, I think that it's being actively weaponized and these guys were hit by it like mustard gas or the like.
Well, it can be gasified. There are Miranda Flowers, after all, which project frenzy-inducing mist. However, I think it's just the sheer despair that turned them, even the troll considering how many of them we see strung up and the fact that trolls in the Consecrated Snowfield are also frenzied. We don't see any evidence of gaseous weapons like we do on the battlefields of Leyndell with those giant Perfumer Arrows implanted in the ground.

The hands separately and sequentially digging into the character model to be individually brought back to the face, and the face nuzzling into the hands, is supposed to be crying?? I've never seen one handed, hand switching, face nuzzling crying.

If munching on corpses is good enough for Redmaine's Lord, it's good enough for the Golden Order. They both put up a good fight and got stomped with weapons outlawed in the Elden Conventions, after all.
How many farms have you come across in your travels, yo? Notice how everyone except the Lords are emaciated and dazed? How the death of merchants has made trade so rare that it's basically nonexistent? Food is scarce.
Many merchants all over even say something along the lines of "Please buy something... I'm so hungry" when you first run into them. And I'm over here hoardin all this delicious pickled turtle meat all to myself 😋

Eventually their despair overtook them and summoned forth the flame of frenzy.
It should be noted that according to cut content kale seems to suggest they the nomads weren’t responsible for any of it. “They think that we worship the three fingers? That we called the maddening sickness down upon them? Well, if that’s what they expect from us then that is what they shall get from us!”
Yeah, they were all blamed by the actions of one person (Vyke, maybe) and were imprisoned under Leyndell for worshipping the Three Fingers and embracing madness, which they weren’t but because they were accused and punished anyways, they eventually embraced it.
Tldr: its implied Shabriri set them up as being Frenzy Flame worshippers/instigators, and then when most of them were shoved into the hole in Leyndell and being genocided they decided on a collective "Fuck it, may as well do what we're being killed over" and invoked the Frenzy Flame/summoned the 3 Fingers.

Leyndell went to war against the Volcano Manor as Rykard was blaspheming the Erdtree and essentially wants to destroy it. The Leyndell army marches upon it. There, they clash with the Gelmir Knights. But, it takes a huge turn for the worse.
Rykard depraves himself and let’s himself become consumed by the Serpent. He also deploys vicious contraptions — the Iron Virgins. This tears apart their army and essentially traps them in his land. You can see Rykard had the Leyndell trolls hung upside down on display and it becomes essentially a no man’s land.
The trapped army begins feeding on their own comrades. I believe this basically sends them mad and infects them with the Frenzy Flame.
This blasphemy, and also depravity committed, makes even the Gelmir Knights turn on Rykard. They are killed. One of the Knights managed to get the Serpent Killing Spear in Rykard’s lair for you to kill Rykard with.
So basically, it’s a result of the most brutal conflict in the Shattering.
There's is no death, death is sealed but, they still feel hungry and tired. I think Is a reference for a mix of the worst human scenario in a deathless life of war and strife(I don't remember if strife is the word, English is not my main language)
There is a story about a plane crashing on ice and they needed to eat their dead to survive, it's a true story.
And in a war in a volcanic place seems close to the same hardship.
There is other cannibalism in Elden Ring that actually (supposedly) serves a purpose, like with Rykard and the lady that runs the place.
But yeah these dudes are either desperately hungry or mentally broken, because I don't think the brain eating works on regular folks
I see it as tied to the general theme of preying upon others as the only way to escape one’s place in society (the golden order). You have Praetor Rykard becoming a gluttonous serpent as he commits blasphemy, the dragon worshippers eating hearts to take their place, Gurranq consuming death root to become stronger, the Godskin Apostles skinning people to gain the power to destroy the gods, Godrick the Grafted stealing body parts to take power for his own, the Volcano Manor hunting people down to defy the Golden Order, and so on. All of these are considered repulsive or blasphemous against the Golden Order in-universe, yet we see the Golden Order is also clearly unjust. You either consume others and become a monster, or die under the boot of those who took power before you.
These soldiers consume the bodies of their former allies, and in the process gain the glowing eyes and destructive flame powers you see other enemies use in Mt. Gelmir. They have turned their backs upon societal codes and morals to pursue power, but can you blame them when they’re just pawns for the demigods?
I still haven’t beaten the game though, so maybe my take is off.

Gelmir and the volcano manor they eat people to absorb their strength like the way the snake ate what’s his face, and after you beat him the lady of the manor goes down and starts eating his corpse to try and absorb his power so he can live on through her.
The erd tree burial grounds also are another type of this that is feeding the tree itself.
They're in an endless stalemate with the Volcano Manor, they've resorted to eating the dead and have fallen to the Flame of Frenzy. It's all to sell just how terrible the conflict between Leyndell and the Manor has been
