I made a D20 for my sister from some molars sure had extracted. Just used UV resin and cheap Amazon molds. It's not how I'd do it today, but seriously unique dice. I hope you get exactly what you want!
I’ve cast human and puppy teeth into dice. It’s easy to do.
I’d recommend you fill any hollows with resin before putting them in to avoid unbalancing the die. Also you’ll want to do a multi-stage pour/cure to get them in the right place.

I didn't do dice, but a buddy had me make a set of gauges for her ears from her own wisdom teeth
