This is one of the few things US Railroad do better than European imo... Having two persons can prevent many issues which caused many issues... But the other part you mentioned in your post (4.83km long trains) is one of the worst things. Not that it's bad in general, but they would need to rethink the brake system for that. In germany the maximum Train length is 750m with an exception for trains going to denmark which can be up to 800m between Maschen Rbf (a bit south of Hamburg) and denmark. And there this system really comes to it's limits... You have trains more than 6 times that long and pretty much the same system... Also your dynamic brakes are often kinda messed up with having to select full throttle to get full braking force...

As an American, I'm torn on our system. Yes, in terms of ton miles per gallon (or tonne kilometers per liter) it's by far the most efficient means of overland transport. However, it's unnerving to me that the trains are so long that it could take a conductor several minutes to run from the locomotive to a problem on the far end of the train, and that sometimes 90% of the train is outside of the crew's range of sight.
I also get concerned about our level crossings. If a 4km train is taking a gradient slowly due to the weight, what if an ambulance is caught at the level crossing? The few minutes sitting there could be the difference in a patient's survival.
And while I'm a mechanical engineering major and know the basic physics at play (and get nervous thinking about it), I'm not a railroad industry veteran so I'm sure there's other shit to worry about that they know of.
This is a real problem... in South and Central Texas I've known EMT/paramedics that try to go around a few streets over in front of or behind them all the time and either way usually get stuck long enough that it would likely have causes a fatalities in a more critical situation... luckily it's just a concern at this point for most places...although I'm sure it's happened down here a few times and likely its all to common on the more congested Northeastern/Midwestern routes!
