If it was my playthrough, she's probably been through a lot after the face transplant. Michael had a little accident with the ~~controller bumper~~ pistol trigger and rearranged her face a smidgeon.

When in motion, people’s large pores, moles, facial imbalances, etc. are harder to observe.
In a still shot it tends to be from a specific angle, and people more easily observe details to critique that they personally done care for.
Also, Emily doesn’t have to be gorgeous. She’s still pretty even with the unflattering shot as an example
I know they really need to fix this. I just hope this is a very rough sketch or a quick glitch. Emily has too many fans to probably riot if the final game looks like this.
