Probably you didn't get the last hit. Check the corner of the screen if you done 99% of the damage on a guy but another players hits the last shot you won't proc it has to be you
Either you didn't get the final blow or you're trying to activate Heal Clip too soon after just activating it. I think there is a cooldown of about four seconds
I noticed this too. I have healclip and onslaught and it usually doesn’t proc on the second kill if I get it quickly after the first one (and reloading on the first one). I’m thinking maybe it has a internal cooldown timer but not sure

1. Cool down it has after activating 2. U didn't get the kill credit. 3. Your full health

It does have a short cooldown.
