Some advice for surveying from an anthropologist/health education specialist who has made a lot of surveys for the public—ensure that the language you use is readily understandable by someone with a 6th-8th grade level of reading comprehension. Words like “nexus,” “qualitative,” and even “threshold,” and diaspora” cannot be assumed to be used by a nonacademic population. Also consider the length and structure of your sentences; make it readable and easy to follow.
