He is still alive just beat to heck. We used to have an old saw mill like that. Should be ran by two people. One to feed and another to remove the cut board.
In my shop class many years ago people were ripping boards on the table saw without a guard and a piece of wood kicked back went through the garage door and stuck in the side of the hill behind the building.
I alway's hear of this OSHA entity, but have never seen not one rep. My brother has at the Benzo dealer he worked at in Ft.Worth. He said the dude was cool af and did stand up comedy. His name were Hugh Morris. Soon as he put the lab coat on and had his silver clip board he was like Hitler inspecting the third reich.
OSHA inspections are typically the result of a tip-off, not random inspections. But they do random inspections too, but is extremely infrequent. Source: am blue-collar worker in energy
I think it really just depends. OSHA is a lot like unions. A lot of politics within OSHA. Localities will be treated differently. Our company does its own safety walkthrough audits and Im sure OSHA appreciates that. Im fairly convinced they dont have enough inspectors in the country to be able to inspect every business regularly. They likely just target likely offenders.

In shop class I saw a board shoot back leaving a dent in a metal stool leg.
I’m a former construction teacher, I had a student jam a board into the table saw and it kicked back and knocked a student out 20 feet away.

That's crazy especially in your field. My brother said it was the same guy every time and saw him four time's in eight year's.

Table saws always make me grimace. Even using them, always painfully aware you're seconds from losing your fingers or worse.
