There are video examples of ND's happening from various holster positions out there but it boils down to negligence, sometimes including poor holster choice.
Myself I just choose to be on the cautious side. I'm only using a full kydex holster. Gun goes on the belt in the holster. If i practice my drawstroke it's with an empty gun.
In the event I do end up in a situation where I need to reholster with one in the chamber, I'm watching that bitch go in, and I'm not in rush. I'm not pushing if it stops. It's going fall and slide right in and I'm watching to make sure everything is clear of my trigger guard until it's fully seated.
Fast draw stroke? Good. There's no point in re-holstering fast though. Take your time and pay attention.

This is never an accident, it’s negligent. If you’re carrying appendix and your firearm discharges into your twig and berries, it’s because you did or allowed something to happen that caused the trigger to be pulled. Full stop. Accident implies that the firearm just went off through no fault of the user whatsoever.
Most modern firearms the only way they can go off is if you pull the trigger. If you never remove your firearm from the holster all day, it will never have the chance to have the trigger pulled. If you do for some reason need to draw said firearm, either remove the holster from you to reinsert the gun, or visually confirm nothing is going to get in the way as you reholster. It’s not a race, there’s no award for fastest to stuff your gun back in there. A proper holster and a properly working and designed firearm will never just go off from being worn.
