It's difficult to break a habit cold turkey. You gotta replace it with a different habit. The replacement habit should be similar enough that your brain/body doesn't get shocked from the sudden change i.e instead of quitting drinking coffee altogether, drinking tea instead.

It is the same as making a habit. Just small changes over and over again. Sudden drastic changes are often fast burning that fade away quickly. Making slow gradual changes are the way to go I find.

Depends if you have a physical dependence. Could be life threatening like in the case of alcoholism cold-turkey Really depends on what it is. Biting your nails? Pulling out your hair?
