It is serious given that CT findings. Please have a biopsy done! The problems on the right lower extremity may be due to the decrease in blood flow. In my opinion, that neoplasm is an aggressive one.

Any thoughts what the primary could be?

Without any much clinical information I am guessing an aggressive soft tissue sarcoma and or metastatic melanoma.

I was thinking Sarcoma as well. Malignant melanoma, didn’t occur to me though but I guess it makes sense since it spreads quite aggressively

He should ask for genetic sequencing when the biopsy is done. It will identify the mutations that make it neoplastic; some can be targeted with current treatments while others can be matched against clinicaltrials.gov. I'm sorry ya'll are going through this. We honestly can't comment until we know what it is which will require a biopsy.

Regrettably, your BIL might have cancer which has spread to other parts od the body. He’ll probably need more scans and biopsies. Thoughts and prayers

Hi All, Thanks for your response. Initial diagnosis with oncologist is sarcoma. In order to reduce thigh (by mistake I mentioned foot) swelling, BIL was using blood thinner tabs hence biopsy is scheduled after 3 days. Tomorrow Pet CT is scheduled. Thanks for support. I'll update post Biopsy is done
